The prompt was benign. The failure occurred during execution.

  • The recently discussed “MOAK (Mother of All KEVs)” exploit chain, shows how multiple known vulnerabilities can be chained together across systems. In agentic environments, this becomes more dangerous—because agents can traverse and execute across those systems automatically, extending the blast radius through the execution path.

  • Research from OWASP continues to highlight excessive agency and tool misuse as primary risks. These are not prompt-layer issues—they emerge when agents interact with tools and systems.

Across these cases, the pattern is consistent:. The failure is not in the request. It is in the execution path.

The Rise of the Agentic Operator Is Not Optional

This is where most companies are still underestimating what is happening.

The rise of the citizen developer was the top-of-funnel event. It created the explosion of builders across the business. What follows is more consequential.

Those same builders are now operating systems that run continuously, make decisions, and interact across tools, APIs, and data systems. These are not static workflows. They are dynamic execution paths producing real outcomes.

A connected view means capturing the full execution path as a single, continuous sequence—linking decisions to actions to outcomes. Without that, there is no reliable way to understand what actually happened. With it, teams can move from reactive debugging to confident operation, reducing resolution time and enabling systems to run at scale.

2. You need causality, not just activity.
Traditional observability captures events. Agentic systems require understanding relationships between those events. Why did the agent choose a specific tool? What context influenced that decision? What downstream actions were triggered as a result? Without causal linkage, teams are left with timelines instead of explanations—and timelines do not tell you how to fix or control behavior. With causality, operators can pinpoint root causes, improve system performance, and continuously refine how agents behave.

3. You need visibility across system boundaries.
Agent execution does not stay within a single service or environment. It crosses models, orchestration layers, MCP servers, APIs, and data systems. Each boundary introduces new context, permissions, and potential failure modes. If visibility stops at any one boundary, the system becomes partially opaque. Operating these systems requires a unified view that follows execution across all of those transitions. That visibility allows teams to safely expand system capabilities without introducing unknown risk.

4. You need real-time awareness of divergence.
The dominant failure mode is not a hard error. It is silent divergence—when the system behaves differently than intended but still returns a “valid” outcome. A tool is called with slightly different parameters. A data source is selected that changes the result. A downstream action executes in a way that was not anticipated. These deviations compound across the execution path. Detecting them after the fact is too late; they must be surfaced as they occur. Real-time awareness allows teams to catch issues early, maintain consistency, and prevent small deviations from becoming large failures.

5. You need control at the point of action.
Control mechanisms that operate before execution (e.g., static policies, prompt filtering) lack the context needed to be precise. Controls applied after execution only tell you what went wrong. The only place control can be effective is at the moment an action is about to occur—when the agent has selected a tool, formed arguments, and is about to interact with a system. That is where intent meets impact, and where intervention can be both accurate and minimally disruptive. This is what enables organizations to scale agentic systems with confidence—without sacrificing speed or flexibility.

This is the shift most organizations have not internalized yet.  This is not about improving observability in the traditional sense—more logs, more dashboards, more metrics.

It is about making agentic systems operable.

And until the execution path becomes the unit of visibility and control, that remains out of reach.
